Severn Health – Living Well Team

At Severn Health PCN our Living Well Team is dedicated to providing compassionate care that enables our patients to live their best lives. We work across the practices offering support tailored to individual needs, ensuring both patients and their families receive the right care at the right time.

Who We Are

Our multidisciplinary team consists of experienced healthcare professionals who work together to provide seamless care and guidance:

  • General Practitioner – Oversees patient care, providing clinical expertise and medical support.
  • Experienced Frailty Nurses – Offer clinical care, health education, and support for managing long-term conditions.
  • Social Prescribing Link Workers – Help patients access community resources, reducing loneliness, improving mental well-being, and enhancing overall quality of life.
  • Care Coordinators – Ensure smooth communication between healthcare services, supporting both patients and their families in navigating the care system.

What We Do

Our Living Well Team provides a wide range of services, including:

  • Personalised Care Planning – Working with patients and families to develop care plans that meet specific health and well-being goals.
  • Supporting Independence – Helping individuals maintain their ability to manage daily activities while planning ahead for their future needs.
  • Social Support & Well-Being – Connecting individuals with community-based activities and services that promote mental and emotional well-being.
  • Family & Carer Support – Assisting families and caregivers in understanding and managing their loved one’s care needs.
  • Coordinated Healthcare Navigation – Ensuring patients have access to the right services, whether it’s specialist care, rehabilitation, or additional social support.

What Happens During a Home Visit?

During a visit our Nurse will visit discuss how you are managing your healthcare and wellbeing. They will complete a comprehensive assessment, which may include memory screening, medication reviews, and a physical examination.

We encourage discussions around healthcare choices, including how individuals want to manage episodes of illness and changes in their health over time. This can include conversations about preferred treatments, medical interventions, and personal goals for
well-being and comfort.

Planning for the Future

As we grow older or experience health challenges, managing everyday activities can become more difficult. Some individuals may have multiple medical conditions or require additional support to maintain their quality of life. Our team helps people take control of their health by working with them to create plans for their future care, ensuring their wishes are respected.

We will ask, “What is important to you?” and tailor our support accordingly. Our team will also discuss personal healthcare choices and future planning to ensure individuals remain in control of their care decisions.


Group & Education Activities

We believe that knowledge and shared experiences can empower individuals to take control of their health. Our Living Well Team offers a variety of group and education activities, including:

  • Health and Well-Being Workshops – Covering topics such as nutrition, exercise, stress management, and managing chronic conditions.
  • Peer Support Groups – Providing a space for individuals to connect, share experiences, and support one another.
  • Carer Education Sessions – Helping family members and caregivers understand how to best support their loved ones.
  • Community Activities – Encouraging social interaction and engagement through group events and activities.

These sessions are designed to provide practical advice, emotional support, and a sense of community, ensuring that individuals feel empowered in their healthcare journey.

Collaboration & Information Sharing

Our team works closely with Volunteer and Community groups, hospitals, GPs, and other health & social care teams. We may share information from our assessments with these teams, but only with your consent and where necessary to provide the best support to you and your family or carers.

Volunteer Led Walks

Walking is one of the safest and easiest forms of physical activity. You don’t need any special equipment to start walking and it can easily be built into your daily routine.
